How to Request a Win/Loss Statement
There are four easy ways to request a win/loss statement from Thunder Valley Casino Resort: online, in person, by mail or by fax.
Once received, requests will be processed and a statement will be mailed to you. Due to confidentiality and privacy concerns, we cannot fax the forms back to you. Please allow up to one (1) week to process your request. Processing may take up to two (2) weeks due to the volume of requests. Illegible or incomplete requests may not be processed.
If you have not already registered for online access to your existing Thunder Club membership, please register here.
Then enter your email and password to access and print your win-loss reports for 2003-2012.

To submit a win/loss statement request form in person, go to the Rewards Center and show your Thunder Club card and valid photo ID. We will print and mail your win-loss statement, W2-G and 1099 summary to you.
